ASP / vbscript: Modify existing web form

I need to modify an existing form on a web page. The existing form does simple Javascript validation, sends an email and saves a record onto the database. I can potentially modify the script myself but I would like to have a professional form better looking and with ajax and css. The modified form has to do the following: 1 Insert a captcha against spam / bots 2 Send an automated email response back to the user/person who has filled the form 3 The email body to be sent back to the user needs to be configurable via a template or include files as follows: --Insert include file 1 --Variable fields from the form, etc --Insert include file 2 So in practice I need to have some thing like this {[login to view URL]} {[login to view URL]} The include files file need to be plain text, the same with the whole email message. 4 Modify the existing form to be validated on the server and add AJAX functionality. 5 Server side script email validation for prohibited domains and prohibited IP's in order to prevent abuse. From an MS Access database, load two different tables as restricted domains and restricted IP addresses. I will provide full details to qualified bidders.
