Client/ Affiliate Management Portals

Im looking for someone to build me a Affiliate/Client Management Portals Below Im going to describe what I need the portals to do on both ends.... Portal 1: Admin Management Portal Across top I want a navigation menu with the following tabs: Home - Active Clients - Suspended Clients - Cancelled Clients - Collections - Add Clients - Schedule Appointments - Affiliates Portal - Earning Reports In the Home tab: When the home tab is hit it should only show the menu across the top and our logo in the body part of the screen. (finish) Active Clients Tab: When we hit the active clients tab it should show us a list of all the active clients first and last name, phone number, and email address. Also when the Active Clients Tab is entered in the corner it should show us a current count of how many active clients we have in the system. Suspended Clients Tab: When we hit the Suspended Clients Tab it should also show us all the suspended clients First and Last Name and phone number. It should also in the corner show a current count of suspended clients we have. Cancelled Clients Tab: When we hit the Cancelled Clients Tab it should show us a list of all the cancelled clients we have in the system their first and last name and also phone number. Also in the corner it should show a current count of cancelled clients. Collections Tab: When we hit the collections tab it should show us a list of all the clients that are in collections with there first and last name, phone number, email, Billing Date, and Monitoring Price Monthly. Add Clients Tab: When we hit the add clients tab it should have the following- 1. Name 2. Address 3. City 4. State 5. Zip 6. Email Address 7. Home Phone 8. Cell Phone 9. Work Number 10. Emergency Number 12. Secondary Contact Person 13. Secondary Person Phone Number 14. Original Contract Date 15. Billing Date 16. Monitoring Price Monthly 17. (Drop Down Box) Active Client - Suspended - Cancelled 18. Client in Collections (Yes or No Check Box) 19. Customer Notes (Text box) 20. Referred Source (It should have a drop down box and a list of every active affiliate we can select to attach to client) 21. Affiliate Commission Pay Amount Schedule Appointments Tab: We should be able to add appointment notes to any day by hitting a button that say schedule new appointments. Schedule appointments tab should have a full month calendar and we should be able to click on any day on the calendar and it show us any appointments we have scheduled right below it. Affiliate Portal Tab: In the affiliate tab it should show a list of all affiliates first and last name and phone number. It should also show a count on how many referred clients that have and also a count on how many active clients they have. It should also show how much commission they have earned in that month as well. Earning Reports Tab: Earnings report tab should show each affiliate and how much they have earned for the current month. Portal 2: Affiliate Management Portal Across top menu we want a navigation menu with the following tabs: Refer New Client - Active Clients - Suspended Clients - Cancelled Clients - Earnings Refer New Client Tab: Refer New Client Tab should have the following: 1. Name 2. Address 3. City 4. State 5. Zip 6. Email Address 7. Home Phone 8. Cell Phone 9. Work Number 10. Notes (text Box) Once Affiliate hits the submit button on refer new client that information is should be sent over to our Admin Management Portal..... Active Clients Tab: Should show all the active clients from our admin portal that only that specific affiliate referred. Suspended Clients Tab: Should show all the suspended clients from our admin portal only that specific affiliate referred. Canceled Clients Tab: Should show all the suspended clients from our admin portal only that specific affiliate referred.
