I have many years experience scripting and designing pdf forms in both Adobe Creative Suite of applications as well as Microsoft Office and I am equally at home designing and working with forms in either of these packages. In the first instance I would need to know in which application was the questionnaire originally designed and if you have the document in that format. Hello there, 7 pages with the functionality you are asking for will take time and adjustments. i am NOT trying to convince you this can be done in a day or two, and in order to make all pages work without problems... i'd create the pages in a layout program (InDesign that would be) and then add/adjust and program the functionality in Adobe Acrobat. One thing, though, that you also have to know is, and I AM telling you this, that there are certain restrictions with saving/emailing filled .pdf's, just telling you so you're aware of this and not surprised and/or wondering thank you (some samples that I created you can view at - replace the 3 "DOT" with an actual . - wwwDOTa1retouchingDOTcom/fillable_pdf_formsDOThtml)
