Collect data from site

I need C# class (you should provide test winforms app) which sends web request and collect data from website. Input param for the class is StationID, (see url in: [login to view URL] [233005, 233006, ... etc] See tab "Follow-up Pass Rate" you need to go via dropdown values of "Scores Applicable as of:" and get information of Station FPR Summary table, for each period in the dropdown output shold be list of items with fields like: string PeriodText : Jan 2013 string PeriodValue: 12/01/2012 string Date: January 10, 2013 - To Present string Score: 0.60 string CertDate: 5/7/2013 string OverallResult: Pass DataTable Table : Table should be in DataTable object with 6 columns like on the page, if no data, then it's empty. You can use HtmlAgilityPack dll in that project.
