Requesting Construction of an Access Database that will keep a small listing of customers, and will allow them to select a plan, and then send a small invoice based on this information.
Access Database must have the following fields
First Name, Last Name, Address, City, State, Zip, Phone Number, Cell Phone Number, Emergency Contact, Emergency Contact Address, Emergency Contact Phone, Notes, Plan Selected.
The Plan Selected will need a drop down box:
One Call Bi Weekly
One Call Per Week
Two Calls Per Week
One Call Working Days
The Invoice will be based on these plans
1 Call Bi-Weekly 5.00
1 Call Per Week 7.00
2 Calls Per Week 14.00
1 Call Working Days 45.00
Setup Fee 30.00
The Setup Fee will be a one time for the first invoice only.
