Hi. I suppose it was hacked by the ssh weak password or with freepbx web if vulnerability. Usually malefactor use some kind of malware and it is very hard to cure and may be impossible to completly remove it without usb live recovery. I suggest to you backup asterisk config and reinstall it from scratch. For protection: 1) Use strong passwords for ssh or use pubkey auth 2) Protect freepbx webif with http auth and use strong passwords, and maybe use https. hackers not like https because it require more resources. 3) Use properly configured fail2ban for protection from sip password cracking.
