Time sheet:
I would like you to make the PDF time sheet into a sheet that automatically fills the fields based on the entered information. All dates automatically fill in when the start and end date are filled in (_________ to _________), Time fields are added for 12 hour time (06:15am ), and all total fields add up respective amount of time.
Basically, I want individuals to fill out the minimum on this time sheet and the form does the rest. Once filled out, form can be saved as a different document.

1) UNMID field is a blank text field
2) Employee Name is a blank text field
3) FTE has options in a drop-down list: 1.0, 0.75, 0.50, 0.25, 0.20, 0.15, and 0.10
0.50 is the default
4) When the date is entered at the top "________ to ________", the first date is also entered on the SAT of the sheet and the last date is entered on the last FRI.
5) All dates are automatically filled in.
6) Time fields are added for 12 hour time in this format: 06:15am
7) Time is automatically sounded to the nearest quarter of an hour, summed, and put in the In/Out Calc column.
8) Blank time fields are put in the REGULAR, OVERTIME, ANNUAL LEAVE, and SICK LEAVE columns.
9) The sum of all hours for the row and columns are put in their respective location.
10) Repeat for Each Row
11) The sum for each typo of hour ( REGULAR, OVERTIME, ANNUAL LEAVE, and SICK LEAVE) is put in each vertical row.
12) Comments is a text field.
Once completed, email me the final document for review and payment.
