Annotations (Note, Text, Rectangle, Line…) at PDF documents

We have a web based document management system developed in .NET.
We use Ghost Script to convert the documents in PDF to display them (viewer).
We want to insert annotations to that PDF documents. We need the same functionality (please also look at the options by right mouse click) as this website:

You have to deliver only the functionality from the left side of the page (annotations). See attached picture ([login to view URL]).
After inserting the annotations user should be able save the document.
Original document or PDF should not be changed by inserting annotations (May be annotations are on a separate layer?).
You will deliver it to us as a full working functionality on a PDF document.
Integration in our DMS is not your job, we will do it by our self. You have to deliver it in a condition that we can integrate it easily in our DMS.
