Build/design a Fitness Website with a form that enters info into an excel doc

Hello, I need a website designed similar to the top of this web template: [login to view URL] Except, the layers don't have to move as your scroll, and I only need the four boxes, I don't need anything below it. Because it is all photos that we will provide, there is not much actual design work to do, mostly just CSS and placement to get it to look like the template above. I need 4 pages: 1) homepage with rotating images (similar to what is shown in template). No flash. Needs to be responsive design. Fonts are large, for senior citizens. 2) About page with similar comparable design 3) Contact Us page - typical contact page with information and also a webform that will send us an email when someone submits it. 4) another form page that will add the info to an Excel document instead of to a database. This would be a simple, just one sheet/table in excel that would add a new row of info each time the form is filled out. There will be no other pages, just links throughout to other websites. Also, I need the content on the page to be in a separate file for each page. This will allow my client to access the wording without having to scroll past (or mess up) the other code. Note: I have a web development company, and we are too busy to take on this project, so you cannot label or advertise this work because you would be a subcontractor to my company.
