Build a Google Apps for work site

Details we are a retailer with multiple locations. I need an app that will allow stores to submit information into a form that is then dropped into a data base that can we monitored. I need to be able to have individual submissions be printed out and updated by others. There are approx 12 pieces of info needed in order for us to preform the task at Headquarters. After the employee has successfully submitted the request the app needs to notify accounting who can then act on it, possibly adding some additional information to the record and changing the status of the record. Once the request is filled I would like the result to be archived where we can generate reports on the activity, I would like the original requester to be notified that the task occurred, I would like the customer to be emailed that the task occurred. Break down Check Request Initiated -Required Data input -Record Established -Accounting is Notified Accounting issues Payment -Record is updated -Status is changed -Record is archived -Customer is notified -Requesting Employee is notified Monthly Reporting -Automatically a recap of activity is generated and emailed to management.
