Create complex form mysql data

User should be able to choose options 4. All entered data should arrive into existing site (PHP/MYSQL) 5. Page needs header, text area, form, footer area. All simple on coloured background. Form needs to look nice from users side. Example 1; User fills in their Name, Email, Contact Number, Creates Password etc then selects a service, then based on the chosen service can choose a package. Upon choosing package the user can chose an add-on. The user can then choose email delivery or postal delivery. The costs for every package, service, add-on, color, delivery is all different. We need to display a message at the end with the total price and simple text instructions that an confirmation email has been sent so please check their email inbox. We need all details saved into our current customer database with the price/package/delivery. Example 2; User fills in their Name, Email, Contact Number, Creates Password etc without choices and simple text instructions that an confirmation email has been sent so please check their email inbox.
