In fact I have successfully completed several similar jobs here in freelancer in the recent past. But I must ask, did you consider using MS Access for meeting your requirements (instead of Excel)? MSAccess is an RDBMS (Relational Database Management System) program, and it shall handle (store, search, report) data much more efficiently and specially in such cases as yours. Excel is good for analyzing a set (or sets) of data, but access is much more efficient in storing, retrieving and reporting on data. Hello, Please allow me to take care of this project :) Depending on your situation I would like to recommend you another possible solution (in case you have a team of experts on remote sites). We could design a dynamic web project based on PHP CodeIgniter Framework to allow registered team-members to fill out various forms from remote locations. All data would be stored in central MySQL database. You would than be able to generate various reports with couple clicks. The best part you could access to your data (protected by username & password) from any location. Just thinking in case you dealing with a lot of emails and similar. It would require more funds of course... However, if you still prefer an Excel form that would interact with Access database (probably) that is doable as well. Of course, depending of amount of data Excel itself can act as a database. There are many different approaches. For example: if you are required to run a report on daily/weekly/monthly basis than we could design a program to combine numerous of excel files and auto-generate report(s). That means each form could be kept as it is (in case you need it that way) in local/shared folder. And not only that, we can also have Excel auto-delivery report via email (in case you use Outlook). I can do a menu in excel, with add-on that will show you how to add to Excel. If there is data that is static (does not change) will make a configuration file where you will be introduced once and can be used continuously with padashtu sheets. You can organize everything in Excel. If I may suggest, we can arrange a separate folder in which you want to create a separate file for each patient.
