I require a form to be made from a couple of files, the files should be self explanatory. The end file in which you will be creating will be filled in using an iPad/iPhone from an operative out on a physical job while inside a property.

PLEASE NOTE THAT THE END RESULT SHOULD LOOK SOMETHING SIMILAR TO THE AS READY PDF ATTACHED HERE

I need you to take the questions from each section from each different document and combine them into one. This will require you to look at the questions and determine duplicate questions. An example would be "How many bedrooms". This question is present in all 3 forms and of course we only need this question asked once. The overall result we are aiming for is to over deliver information on properties, very quickly, efficiently and digitally rather than paperwork. Combining all the questions across 3 forms into 1 will enable this to happen.

Although most of the questions are text based here I require drop down boxes for almost everything EXCEPT the questions on the form with radio check boxes. These questions have few answers like "Type of Property" only has 5 or 6 answers so in the interest of this form being a speedy form to fill in please apply to all those questions you see fit.

The questions that will require drop down boxes I will edit the selections to add whatever choices the question may have myself, I have LiveCycle and FormCentral so don't worry about that.

The form should have room for some type of logo and footer. The form should allow to easy editing, if you see fit include a quick guide on how to edit it. As stated it should be clean, the information be clean on delivery and it be easy to read. IT will need to operate with an iPad/iPhone.

Features I would like it to have are,

Auto dating the PDF file with the date and time on the deivce

A checkbox, when ticked it fills in date and time next to it once clicked. For when you walk into a property "In Possession (checkbox)". When clicked "In Possession 09/01/2014 9:04am"

The easier you can make it to add and remove questions the better, this job is a relativly simple job so bid accordingly. I will be accepting genuine bids quickly. This is an edited version of a previous more complex project.
