Create a professional, easy to use electronic data entry form from a current 10 page list of questions. Data will be entered by keyboard or touch screen (ultra laptop or tablet.) Check boxes, drop down menus and other quick entry features important. Form must be created with common used form creation software so that a typical computer user could make changes and future updates as needed. It would be very useful if the date collected could eventually be used to automatically populate a narrative report.

Hello There, Greetings if the day I can develop a dynamically managed Responsive form for you. As soon as someone fills up the form you and the applicant will get the confirmation mail. I can also make it in to integrate Phone sms. Also all the dropdowns can contain master data for example list of countries, states, categories etc Also you can manage these lists available for users through admin panel.
