Crime Tip Database & Website

I am looking for someone to build a database for a crime tip website. I need the website built complete with domain/hosting and the database. In short, my website would consist of a tip reporting page with drop down menus for city & state, type of crime that is being reported and text boxes that allow user to enter in information such as location of crime, suspect information, vehicle information etc. I have already created this page on Microsoft publisher so it just needs to be converted, (email me with your email address and I will send to you). When a person is reporting a confidential tip of a crime, the information is stored into the database and is accessible by law enforcement agencies (with username and password). Each registered user would enter their user name and password and be able to access any tips reported in their respective city or department. An email is also sent to the user stating that they have new tips. The tips are generated and printed in a report format. The website and database should have admin link to easily manage both.
