Allergologist needed for therapy recommendations

I have a list of 54 allergies that I want to write medical treatment recommendations for. Something ins the style of this: Patient sensitized to the two mite major allergen groups responsible of persistent respiratory symptoms such as cough, hindered respiration, asthma. This clinical picture is exacerbate during the pollination of grass to which patient resulted also sensitized. Treatment The treatment of choice is this co-sensitised patient is Sublingual Immunotherapy (SLIT). 1) Staloral 300® MITE extract, Stallergenes. A maintenance dose once a day all over the year. 800 microliter/day is the dose suggested. The SLIT has to be self administered at home. The daily dose must be held under the tongue for 3 min before swallowing, early in the morning before breakfast. SLIT has to be continued for 4 years. The daily dose must be held under the tongue for 3 min before swallowing, 2) Grazax®, ALK(or ORALAIR®, Stallergenes), 1-2 tablets for 3-4 months (January-April), early in the afternoon. SLIT has to be continued for 4 years. 3) In case of symptoms I suggest Cetirizine (Zyrtec): 8-10 drops/24h (before sleeping) for 2-4 months. Singulair 5 mg (montelukast) 1 tablet at 6 pm for 2-4 months. Seretide Diskus 100 once-twice/day for 1-2 months at the peak of pollination. I am looking for someone who can determine the therapy of choice for this list of common allergies so I can automate the reporting. Allergy background required!
