ASP.NET Storefront Custom Forms Entries to MSSQL

We have an ASP ([url removed, login to view]) website on a Windows/IIS server with a local MSSQL database. We need a custom form added to the product page templates where customers can add/attach their information to the product they are purchasing. Once the purchase is completed (payment processed), the customer information will be entered into the website database but it also must be inserted into a another, more secured database using SOAP calls.
