Hello, Sir/Madam. I have a few questions that I would like clarified:

1. How many forms does the app require? Would you like the HTML form placed in an app only or are we expected to create the forms shown in the images?

2. What exactly is required of the app? Please correct me if I am wrong but from what I could gather, the app is supposed to allow users to enter some information into a form (or multiple forms depending on answer to 1 above) and send that information somewhere. Is this correct?
